fa ارتباط بین درک مراقبت بیهوده و پریشانی اخلاقی در پرستاران بخش مراقبت‌های ویژه بیمارستان‌های آموزشی دانشگاه علوم پزشکی مازندران سال ۲۰۲۶ The Relationship Between Perception of Futile Care and Moral Distress Among ICU Nurses in Teaching Hospitals of Mazandaran University of Medical Sciences 2026 عمومى General پژوهشي Research مقدمه: بخش‌های مراقبت ویژه (ICU) به دلیل پیچیدگی‌های بالینی و نیاز بیماران بدحال، پرستاران را در معرض موقعیت‌های اخلاقی دشوار قرار می‌دهند. یکی از چالش‌های اصلی در این بخش، ارائه مراقبت‌های بیهوده است که نه کیفیت زندگی بیماران را بهبود می‌بخشد و نه وابستگی آنان به مراقبت‌های پیچیده پزشکی را کاهش می‌دهد. این وضعیت می‌تواند منجر به پریشانی اخلاقی در پرستاران شود. هدف این مطالعه بررسی ارتباط درک پرستاران ICU از مراقبت‌های بیهوده با میزان پریشانی اخلاقی آنان بود. روش‌ها: این مطالعه توصیفی–همبستگی در سال ۱۴۰۴ بر روی ۹۷ پرستار شاغل در بخش ICU بیمارستان رازی قائم‌شهر، وابسته به دانشگاه علوم پزشکی مازندران انجام شد. معیارهای ورود شامل داشتن مدرک کارشناسی یا کارشناسی ارشد پرستاری و حداقل ۶ ماه سابقه کار در ICU بود. داده‌ها با پرسشنامه مراقبت‌های بیهوده و پرسشنامه آشفتگی اخلاقی جمع‌آوری شدند. روایی و پایایی ابزارها پیش از مطالعه تأیید شد. داده‌ها با استفاده از SPSS نسخه ۲۱ و آزمون‌های آماری توصیفی و استنباطی غیرپارامتریک شامل مان–ویتنی، کروسکال–والیس و ضریب همبستگی اسپیرمن تحلیل شدند. یافته‌ها: میانگین نمره درک از مراقبت بیهوده 57/14 + 5/ 45  بالاتر از میانگین و میانگین نمره پریشانی اخلاقی  85/7 + 75/44 نیز بالاتر از میانگین به دست آمد. ضریب همبستگی اسپیرمن ارتباط معنی داری را بین دو متغییر نشان داد(01/0 >P، 39/0= r)همچنین افزایش سن و وضعیت تجرد با افزایش درک مراقبت بیهوده و پریشانی اخلاقی مرتبط بود، در حالی که جنس و سابقه کاری تأثیر معنی‌داری نداشتند. نتیجه‌گیری: یافته‌ها نشان می‌دهند که پرستاران ICU که درک بالاتری از مراقبت‌های بیهوده دارند، بیشتر دچار پریشانی اخلاقی می‌شوند. این نتایج بر اهمیت طراحی مداخلات آموزشی و حمایتی و ارتقای فرهنگ سازمانی برای کاهش پریشانی اخلاقی و بهبود کیفیت مراقبت بیماران در ICU تأکید می‌کند.   Background: Intensive Care Units (ICUs) expose nurses to complex clinical situations and challenging ethical dilemmas due to the critical conditions of patients. One of the main ethical challenges in these units is the provision of futile care, which neither improves patients’ quality of life nor reduces their dependency on complex medical interventions. Such situations can lead to moral distress among nurses. This study aimed to examine the relationship between ICU nurses’ perception of futile care and their level of moral distress. Methods: This descriptive–correlational study was conducted in 2025 on 97 nurses working in the ICU of Razi Hospital, Qaemshahr, affiliated with Mazandaran University of Medical Sciences. Inclusion criteria were holding a bachelor’s or master’s degree in nursing and having at least six months of ICU work experience. Data were collected using the Futile Care Questionnaire and the Moral Distress Questionnaire. The validity and reliability of the instruments were confirmed before the study. Data were analyzed using SPSS version 21 with descriptive statistics and non-parametric inferential tests, including Mann–Whitney, Kruskal–Wallis, and Spearman’s correlation coefficient. Results: The mean score of nurses’ perception of futile care (45.57 ± 14.57) and moral distress (44.75 ± 7.85) were above average. Spearman’s correlation showed a significant positive relationship between the two variables (r = 0.39, P < 0.01). Additionally, higher age and being single were associated with higher perception of futile care and greater moral distress, whereas gender and work experience had no significant effect. Conclusion: The findings indicate that ICU nurses with greater perception of futile care experience higher levels of moral distress.
