Background :Idealism, as one of the classical philosophical schools, emphasizes the primacy of the mind, ideas, and moral values, and can provide a conceptual framework for the analysis and design of health systems. An effective health system requires value‑based governance grounded in ethical and spiritual principles, which idealism highlights as a fundamental pillar for improving the quality of nursing and caregiving services. This study aims to examine the foundations of idealism and its influence on the goals, structure, and performance of health systems. Methods: In this narrative review, published articles up to the year 2026, without any time restrictions, were examined. To identify relevant scientific evidence, an electronic search was conducted using predefined Persian and English keywords across the following databases: Scientific Information Database (SID), Magiran, IranMedex, PubMed, Scopus, Ovid, ScienceDirect, Elsevier, ProQuest, and Google Scholar. Articles were selected based on predefined inclusion criteria, a stepwise screening process, and exclusion criteria. Ultimately, nine relevant, credible, and analytical sources were included for the final analysis. Findings: A review of the sources shows that idealism as a philosophical school has had an impact on the formation of health system goals and policies. The findings of this review can be summarized in three axes: ethical principles, justice in health, and health policy-making. The implementation of idealism in health faces problems such as high costs, consideration of profit-oriented economic issues, and implementation challenges. Conclusion: Health idealism provides a criterion for a centralized system, combining it with practical and realistic policies to achieve needs-oriented. Data management policy, the role of technology, and professional ethics education are suggested as ways to achieve the goal.
