Background: Intensive care unit (ICU) nurses play a vital role in delivering critical and safe care to critically ill patients, and the quality of services they provide is influenced by both individual and organizational factors. This study aimed to examine the relationship between job autonomy, organizational support, and job motivation of nurses with care quality indicators in ICUs of teaching and treatment hospitals affiliated with Iran University of Medical Sciences. Methods: This cross-sectional study was conducted among ICU nurses. Data were collected using standardized questionnaires assessing job autonomy, organizational support, and job motivation. Care quality indicators, including patient falls, surgical site infections, pressure ulcers, and adverse drug events, were evaluated. Data were analyzed using appropriate statistical tests and logistic regression. Results: The findings indicated that nurses reported moderate levels of job autonomy and job motivation, while perceived organizational support was low. The incidence of care quality indicators over the past six months ranged from 26.2% to 49.5%. A significant association was observed between organizational support, job autonomy, and job motivation with care quality indicators. Specifically, higher organizational support and job motivation were associated with a reduction in adverse events and improvement in care quality. Conclusion: Strengthening individual and professional factors of nurses, including job autonomy, organizational support, and job motivation, can enhance care quality and improve patient safety in ICUs. The results highlight the need for organizational support and educational programs, as well as promoting professional autonomy, to improve performance and reduce adverse events in clinical settings.
