Introduction:Nursing documentation is one of the essential components of safe and high-quality patient care and the most important communication tool within the healthcare team. Despite its importance, evidence indicates that the quality of nursing documentation in many healthcare centers is not satisfactory. In-service training programs can play an effective role in improving this situation. This study aimed to evaluate the effectiveness of a nursing documentation training course for nurses at 5 Azar Hospital in Gorgan based on the Kirkpatrick evaluation model in the year 1404 (2025). Methods:This study was a descriptive cross-sectional research conducted using a census method on 290 nurses working at 5 Azar Hospital in Gorgan. In the first stage, 300 inpatient medical records were randomly selected and reviewed for documentation errors using a standard nursing documentation checklist. Based on the identified problems, an 8-hour educational workshop was then held in accordance with the Ministry of Health curriculum. The effectiveness of the training course was evaluated using the four levels of the Kirkpatrick model: reaction, learning, behavior, and results. Data were collected through questionnaires, pre- and post-tests, performance checklists, and a re-review of medical records, and were analyzed descriptively. Results:At the reaction level, overall participant satisfaction was reported as 93.59%. The mean pre-test score was 82.74 and the post-test score was 84.72, indicating an improvement in nurses’ learning after the training. At the behavior level, the mean performance score of nurses was 97.59%. At the results level, the mean rate of documentation errors decreased from 14.18% before training to 6.56% after training. The final effectiveness index of the course, calculated based on the weighted mean of the four Kirkpatrick model levels, was 95.92%. Conclusion:The nursing documentation training course demonstrated high effectiveness and led to improvements in nurses’ knowledge and performance, as well as a reduction in documentation errors. The use of the Kirkpatrick model is recommended as a comprehensive approach for evaluating in-service nursing training programs to enhance the quality of nursing care.
